Maybe some higher level standard items could be available in the donor zone, like mystic staves, addy/rune bolts, dragon arrows, etc, and possibly a potion store? Maybe someone who sells cut gems as well. Maybe you could add a few custom colored party hats and make them donator then make the default ones extremely rare boss drops or vice versa. I hate when all the 'rares' are donor only.
As for customs I've seen servers with 12 sets of custom armor, but they are all just different colored torva, I would like to see lots of custom armor that isn't simply the same recolored set. It doesn't all have to be totally new, One of the better servers I played had customs that were all recolored barrows items where the armor pieces were miss-matched but recolored to belong to new sets, like a blue Verac's helm, blue Dharok's body and a blue Torag's legs to make a "water set," etc. Slayer helm should also be added.

Yeah we've decided for donor pretty much what you said besides the rares, but I agree we could implement it once we have customs ingame.
I think custom items that are unique to be more important, I do like the barrows to water set idea because some combinations would look cool. I think the kc ring may be the first although it will use a model we currently have
